Admissions Forms Descriptions

The Admissions Forms folder in Galaxy contains forms used to submit documents or make requests concerning your application to the Office of Admission and Enrollment. See the descriptions below to determine the appropriate form to use to submit your documents/requests. The forms you see may vary based on your level and current status – not all forms will be visible to all applicants.

Accessing the Admissions Forms

To access any of the admissions forms, log in to Galaxy, then click Admissions Forms Folder under the Orion Self-Service (Student) menu.Select Admissions Forms Folder under the Orion Self-Service menu.

Select the relevant form:

There are a number of forms available within the Admissions Forms Folder.

Fill out the required information, such as in the sample form below, and click Submit.

Admissions Forms Descriptions

  • Submit Educational Records – this form can be used to submit an unofficial copy of your academic records such as transcripts, degree certificates, admission test scores and English proficiency scores.
  • Submit Supplemental Docs – this form can be used to submit a copy of your Letter(s) of Recommendation, Essay, and/or Resume.
  • Submit App Fee Waiver – this form can be used to submit documents needed to waive your application fee.
  • Submit Residency Document – this form can be used to submit documents needed to support your residency status determination.
  • Change of Application Request – Applicants who have not been admitted to UT Dallas can use this form to request to change the program/plan/term on your application.
  • Application Docs Cover Form – this document is optional and applicants can print and complete to attach to any documents you submit via mail to the Office of Admission and Enrollment. The information provided on this form will only be used to match your documents to your application and may help speed the processing of these documents.
  • Request Graduate Deferment – Graduate level students admitted to UT Dallas can use this form to request to defer their admission to a new term.
  • Request Undergraduate Deferment – Undergraduate level students admitted to UT Dallas can use this form to request to defer their admission to a new term.
  • Submit Meningitis Vaccine – this form should be used to submit a copy of your Meningococcal Meningitis Vaccine Requirement Form and supporting immunization documentation.
